The study questionnaire is provided for informational purposes ONLY.  If you are participating in the study or are interested in participating, please feel free to review the questions.  This form is NOT to be filled out and mailed or emailed.  All questionnaires must be completed by a member of the SBRR staff during a phone interview. We cannot accept or use any questionnaires returned to us filled out.

·        For individuals who have signed a consent form and are currently enrolled in the SBRR:  A member of the staff will interview you by phone to complete your questionnaire.  You are welcome to print out the form and jot down your answers before the phone interview but do not return a completed form.

·        For individuals who are interested in participating in the SBRR: Please contact one of the study coordinators to discuss what participation in the study involves and the rights of study subjects.  You may contact us by phone or email or you may fill out the interest form and mail it to us.
